How does AviClear™ work?

Acne forms when sebum, the oily substance on your skin, combines with dead skin cells and clogs your pores. Most acne treatments target other things that cause acne, but not the production of sebum.

What to expect

Is AviClear™ painful?

AviClear™ uses a skin cooling sensory control called AviCool™ sapphire that is designed to keep the skin at a regular temperature during treatment and therefore a more comfortable and safe experience for our patients. We do offer other means of pain relief, for example, a water-based numbing cream, which is discussed further in your initial consult if required. Patients who undergo AviClear™ may experience a sensation like a slight snap or what has been described as a static shock during the treatment.

The treatment is patient driven and centred, we have created a welcoming and comfortable environment to ensure patient comfort.

What is the downtime?

Some patients may experience swelling with or without pustules for as little as a few hours or up to a few days post-treatment. Dryness and itchiness can occur that may last a few weeks, which is where the importance of skin care to the patient will  be noticed. Acne flare ups or 'purging' has the potential to occur after each AviClear™ treatment.

Having a good understanding of skin care is important after treatments to decrease irritation, and prevent scratching and potential of acne scarring. This will be discussed in your initial consult and we provide support and skin care options if the patient does not yet have their own regime.

Results are typically seen at least 3+ months after the third treatment. This timeline can differ per patient and results can be seen at 6 months with further improvement at 12 months.

Aftercare

Having a good understanding of medical grade skin care and a personal routine is required in your aftercare. This will be discussed in your initial consultation where support is also provided if the patient wants to begin or change their skin care regime. What to expect

During treatment, you may feel some discomfort, typically described as a brief sting. Afterward, you might experience localised redness and swelling, followed by a temporary crust formation that naturally exfoliates in 1-4 weeks.

Number of treatments

While some concerns vanish instantly, larger irregularities may require 1-3 treatments, these treatments are usually spaced 3-5 weeks apart.

Aftercare

• Avoid harsh skincare products for 14 days.

What is Photodynamic Therapy (PDT)?

Photo Dynamic Therapy is a medical treatment that uses a combination of a photosensitising cream and specific wavelengths of light to treat various conditions, including superficial basal cell carcinomas (BCCs) and squamous cell carcinoma (SCC) in situ (Bowen’s disease).

Two treatments are required, seven days apart. A cream is applied to your skin, before being activated by an LED light. The activated cream selectively destroys only the skin cancer cells. This means that side effects such as scarring are minimised and healthy tissue surrounding the lesion is not affected.

PDT can produce excellent cosmetic improvements, as well as removing the problem cells. It is used when other treatments or surgery are considered unnecessary or unacceptable.

How does Photodynamic Therapy Work?

• Administration of Photosensitising Agent:

Patients will have the lesion prepped and the cream applied. The cream is not active until it is exposed to LED light.

• Absorption of Photosensitiser (cream):

The photosensitising cream is absorbed by the targeted cells, accumulating selectively in the abnormal or cancerous tissues. A wait time of three hours is required.

• Irradiation with Light:

After the three hour incubation period, the drug is absorbed, the targeted area is exposed to light of a specific wavelength. This activates the photosensitiser, leading to the production of a photochemical reaction.

• Cell Destruction:

The cancer cells absorb the light sensitive medicated cream. A light is then applied to the area where the cancer is. The light causes the drug to react and produce a type of oxygen that kills the nearby cells. 

Advantages of Photodynamic Therapy

• Minimally Invasive:

PDT is a minimally invasive treatment, reducing the need for extensive surgery.

• Targeted Treatment:

The photosensitising cream selectively accumulates in the targeted cells, minimising damage to surrounding healthy tissue.

• Reduced Side Effects:

Compared to some traditional treatments, PDT often results in fewer side effects.

• Versatility:

PDT can be used to treat a variety of conditions, making it a versatile therapeutic option.

What is suitable for PDT?

• Superficial basal cell carcinomas (BCC's)
• Bowen's disease (squamous cell carcinoma (SCC) in situ)
• Actinic keratosis (solar keratosis)

PDT is not suitable for deep basal cell cancers or squamous cell skin cancers. This is because the light cannot go deeply enough into the skin.

During the treatment

You may feel a stinging or burning sensation in the treatment area when the light is on. A fan or painkillers can be used to make the treatment more comfortable. 

You can have more than one skin cancer treated at a time. 

After treatment

A scab forms where you had the treatment and you have a dressing in place to cover it. A steroid cream may be applied on the treatment area. This helps to reduce any itching.

The scab usually heals in about 3 weeks. 

The light sensitising cream makes the treated skin more sensitive to sunlight. We recommend that you wear sunscreen of SPF 50+ for at least 72 hours.  You might need to avoid bright light or sunlight for a time. This is because all your skin may be more sensitive to sunlight.  

Side effects of PDT

The treated area might have some inflammation. It may look pink, red and swollen. The site might also ooze or develop a crust. This should get better after a few days. But if you are worried contact Hobart Plastic Surgery.

All LED light sessions of PDT take 20 minutes, and all have a final review appointment by our surgeon after 3 months (bulk billed appointment).
